    Prior to any student participating in a sport, the student is required to complete registration for that sport through FamilyID.com
    Along with registration for the first sport of the school year a PIAA Physical Evaluation form must be completed with Physician Signature and Stamp (forms in link below).
     
    Any athlete competing in subsequent seasons for a school year must also register for their in-season sport  and then submit a PIAA Re-Certification form signed by the Parent or Guardian (forms in link below).
