Beginning with the 2008-2009 school year, the PIAA has enacted a new Pre-Participation Physical Evaluation and Parental consent process.  The Parent/Physician Certification cards of years past will no longer be used.  Parents and Physicians will now need to complete a four page Comprehensive Initial Pre-Participation Physical Evaluation for each students first sport in a school year.  As in the past, the physical examination required may occur no earlier than June 1^st.  Subsequent sports participation in the same school year will require the submission of Re-Certification documents by parents and physicians.  Forms are available in the school offices.
