Independence Blue Cross - Personal Choice Program
Employees are provided with a Personal Choice 10/20/70 medical and prescription plan. Employees can find doctors, manage their healthcare and much more on the Independence Blue Cross website: www.ibx.com

The benefits summary below provide you with indepth information regarding the available plan.   
 
Prescription coverage includes a $10.00 co-payment for generic drugs and a $25.00 co-payment for brand-name drugs.

 

Dental Coverage-United Concordia Flex

Overview of UC Flex Program:
Routine Services (deductible waived for level 1):  100% Coinsurance  for: Oral examinations, X-rays, Prophyalxis, Fluoride Treatment.
 
Restorative Service: 80% Coinsurance  for: Amalgam, porcelain, plastic filings, extractions, space maintainers.
 
Prosthetic Services: 80% Coinsurance for: Crowns, bridges, dentures, endodontics, gold inlays, periodontics.
 
Orthodontics (limited to dependents under age 18): 50% to $1,250

Maximum Benefits: $1,400 per dependent
